custom form to enter detail chose template and checkout via paypal

hi anybody can help me out

i need a form that user can enter their detail choose a selected template and checkout via paypal.

i will be integrating this form in Joomla.

i know how to build a simple form but have no idea how to put the template part and also how to include paypal payment to it.
