can i insert filename into database with uploading file with plupload?please tell me i am new to plupload.

Yes. In the upload.php in the download package you can see how to get the filename. Just add your own query.

Wherever you want. It depends on what you want to do. This file is executed for each uploaded file. So, if you want to add something once, it would be best to use a form submit to a new page. I think the demo (as it is in the zip) POSTs to the dump.php script (it's been a while since I've built my CMS with this).
