When I click submit the database was showing the date with 0000-00-00 . need to entry full data into database.

The html form date type is in most cases just a text.
If you want te curent time justuse the php date function.
if you want another date I recoment diverent fields for day, month and year.
Also: mysql is deprecated use mysqli.
Also 2: check out mysql_real_escape_string (or even better mysqli_real_escape_string) to prevent hacking

Is there any reason why you're dumping 100's and 100's of lines of code? If you have a problem, please outline its nature and only include the relevant bits of code. For example, why dump all that css? Totally pointless.

Your form states start and enter - shouldn't that be start and end?

Also the input has the name "date" for start. The is no datepicker for enter/end.

So I can't work out this...

$term = $_POST['year'];
#$term = $_POST['month'];

The second line is commented out for some reason but neither exist anyway.

So this:

$sql = mysql_query("SELECT * FROM entry WHERE species LIKE '%".$_POST['species']."%' && year LIKE '%".$_POST['year']."%' && month LIKE '%".$_POST['month']."%'");

Will be error-strewn.

no this is not the error,

need to change all sql data. for those term are not available

in the data base I saved the date data as 2014-02-03"yyyy-mm-dd"

I'm afraid you're not making things easy here. The sql will not work. I have shown you where your variables are problematic. Unless you can get hold of valid input data, your sql won't work.

In addition, you do not clean/escape your input data, so you are vulnerable to sql injection.

Your SQL should look like this:

$sql = mysql_query("SELECT * FROM entry WHERE species = '$species' AND YEAR(datefield) = $year AND MONTH(datefield) = $month");

*datefield = the name of your date field in the db table.

The variables should be cleaned inputs. But as I tried to tell you the year and month cannot be extracted from the datepicker on the form.
