What do you think the answer to no.1 is?
The answer to no.2 is in the AMD and INTEL manuals of course and plenty of docs on the net.
Do you know what a "register" is? All it is, is memory directly on the CPU die, which makes it the fastest memory to access.

2.) The process which goes on within the processor to figure out what a machine code instruction means.
3.) A register is just a small area of memory within the orocessor itself.
