Unable to Insert correctly into the database, please help

I am a newbie, really need your help. I've been working on this for 2 days and can't get it to work correctly. I have a form that I want to submit to the database "New Lead" form. I am using the submodal feature for the customer notes. For some reason, the data is not inserting correctly into the database, I keep getting an error which says undefined index "Notes." Here is the code that I am using for the table form, and also the insert statement...please help!

Your form appears not to have an input named "Notes" - that's the index PHP says you're missing, and that might be the problem.

Right, I am not sure how to set that input, since I am using the submodal feature....

I'm sorry, I'm not very well acquainted with the submodal feature. The only thing I can tell you, is that I do not see an input field with the name "Notes" - which is required in your insert query.

as

<td>Notes: <input type="text" name="Notes" id="Notes" /></td>
