Can't you find this code on the internet? Google is really good - you just type in a few words, and amazingly it gives you a list of web pages that you can investigate. I have found that using this, I think they call it a search engine thingy or something, has saved me a lot of time.

If you can create list box in plain HTML, then you should be able to do it in PHP too. Simply echo them.
