Every time I start up my system I get an Application failed to initialize properly (oxc0000005) error. seems to come from a 9vl8ya1a.exe file which I cannot trace. Any suggestions?

Could be malicious code. Try downloading Ad-Aware and Spytbot S&D. Run them to clean your computer of adware and spyware then try it again. If that doesn't work then I'm out of suggestions, except formatting... heh.
